Statewide Preconception/Interconception Demonstration Project Grants
Los Angeles, San Francisco and Yolo Counties
California Family Health Council (Second year funding)*

This project will assess the delivery of preconception/interconception care at Title X Family Planning Clinics in San Francisco, Los Angeles and Yolo counties. Upon completion of the assessments, a curriculum will be developed and training will be held within each clinic.  An evaluation will be conducted to measure the effectiveness of the training as it relates to clinician and client adoption of preconception/interconception concepts. A cost allocation model associated with the delivery of these services will be developed and used within the context of policy change and program sustainability.

Los Angeles County
Harbor-UCLA Medical Center (First year funding began October 2007)**
The Maternal-Child Cohort Intervention project will enroll women with gestational diabetes/pre-diabetes in a coordinated maternal-child preventive care program to promote: i) her role in establishing healthy lifestyle to control/prevent diabetes and achieve healthy weights for herself and her child; ii) breastfeeding during the first six months of life; iii) effective, safe contraception and diabetes control/prevention during interconception; and iv) planned pregnancies in optimal glycemic control.  The project will use case management to implement best practices and preventive care protocols by coordinating existing separate medical and social services.  
** Project was funded for Year 1 at $100,000 and will be re-evaluated in September 2008 for Year 2 funding.

PHFE-WIC Program (Second year funding)*

The WOW – WIC Offers Wellness project will: i) explore Los Angeles County WIC data to locate neighborhoods with high rates of prematurity and low birth weight in WIC mothers; ii) identify and train a Care Coordinator to implement care coordination strategies/activities/protocols for postpartum WIC mothers, iii) design and implement a database such that, for the first time, WIC women may be tracked during their interconception period and iv) evaluate the impact of WOW by comparing women receiving WOW services with women at a matched control site.

Sutter Teen Programs (Second year funding)*

The Teen Care project provides interconception risk assessment and intervention through home visitation to pregnant and parenting teens with high risk medical and/or social risk factors or with prior adverse pregnancy outcomes.  The goals of the project are to achieve decreased adverse birth outcomes; positively impact changeable health behaviors and create a replicable package of interconception services for pregnant and parenting teens that includes innovative, culturally appropriate resources such as on-line risk assessment tools. 

* These projects are proposed to run for three years but continuation funding will be determined annually based on a review of project progress.

Contra Costa Health Services

The Bridges to Prenatal Care for Substance-Using Women Project is designed to decrease the number of substance-using women who deliver in Contra Costa County with late or no prenatal care and to increase the utilization of prenatal care services among this population of women.  The project will include two main strategies: 1) preparing the prenatal care, Labor and Delivery, and NICU systems in Contra Costa County to work more effectively and knowledgably with substance-using women who fear being reported to CFS and losing their children, and 2) implementing a consumer-designed community awareness campaign with clear, accurate, and encouraging messages about what substance-using women can expect when they seek prenatal care. 

Fresno County
California Health Collaborative

This project will provide individual and group psychotherapy for women who are pregnant and have diabetes. A Marriage and Family Therapist-Intern will be used to provide individual therapy sessions with one hour of supervision of the intern for every 10 clients seen, as required by law.  CHC will be offering individual therapy at The Diabetes Care Center with up to 24 individual sessions available per client. Group Therapy services to neighboring hospitals will also be provided to address sustainability and provide for the future growth of psychosocial services for mothers with diabetes.

Paso a Paso is a culturally competent educational program for Spanish speaking women and their families.  This comprehensive program prides itself on improving access to adequate and early prenatal care, improving newborn birth weight, and improving successful long-term breastfeeding.  March of Dimes funding will help to replicate the program in the Southern part of the County servicing the Eel River Valley and Redwood Memorial Hospital.

LA Best Babies Network

The LA Best Babies Network aims to improve the quality of culturally competent prenatal care and significantly reduce risks of low birth weight, preterm birth and complications of chronic conditions, such as diabetes and hypertension, in regions with the highest racial and ethnic perinatal disparities in Los Angeles County.  This project will disseminate information about CenteringPregnancy® and coordinate provider training and adoption of this innovative model for providing group prenatal care.

University of Southern California, Center for Premature Infant Health & Development

This project is an education and support intervention in African-American churches to increase the capacity of churches and individual childbearing women to promote healthy pregnancy via church-wide activities/events, social/emotional support, and health education.  The Center will partner with churches and key agencies in Los Angeles to provide technical assistance, health advocacy training and health education to the church leadership and congregations.

San Diego County
San Diego County Health and Human Services

The Preconception Health Awareness Project will target all women of childbearing age in San Diego County, with a focus on African-American and Latina women.   The project will develop and distribute culturally and linguistically appropriate materials to educate women of childbearing age about preconception care.

Santa Clara County
Santa Clara County Black Infant Health Program

The BIH Well (pronounced Be Well) Project will promote positive behavior change in African-American women by improving physical fitness, nutrition and overall well-being. The project will provide classes that combine education and exercise in weekly meetings.   Participants will receive support to overcome barriers to wellness goals and the challenges of post-natal fitness.  The aim of the project is to empower women with the tools, strategies, and resources needed to provide healthy, nutritious meals and consistent beneficial exercise habits for themselves and their families.

San Francisco County
Southeast Health Center

The Healthy Mom - Healthy Baby program aims to increase the current low rate of early and adequate prenatal care received by women living in the isolated, primarily poor and African-American Bayview-Hunters Point community of San Francisco.  The project will implement a perinatal patient navigation program to enhance prenatal care at the Health Center.  The navigation program will hire a support worker to help prenatal patients overcome barriers to care and to assist them with attending their medical, social work and nutrition appointments at the Health Center and at other locations.
